Consular Mission to Cleveland, Ohio December 6-7th
Consular officers from Lithuanian Consulate General in cooperation with the Lithuanian American Community Ohio Region and Lithuanian Honorary Consul General Ingrida Gertrūda Bublys will be providing Consular Services in Cleveland.
Consular services will be provided at: Lithuanian Community Center, 877 East 185th St; Cleveland, Ohio, 44119.
During the Consular Mission it will be possible to submit applications for passport renewal and issuance, forms for registering of the civil acts in Lithuania (marriage, divorce, birth of children), declarations of the place of residence, and applications regarding Lithuanian citizenship.
In order to receive consular services you have to register in advance at the Consulate General of Lithuania in Chicago by e-mail: kons.cikaga@urm.lt
To register, please provide the following information:
1) Your full name - exactly as it is written in your passport. If your name changed due to marriage or other circumstances- please provide both- your previous name, and your current name.
2) Personal code (asmens kodas), or your exact date of birth (in case person does not have personal code yet).
3) Your contact information (phone number, E-mail address).
4) What consular services you would like to apply for.
5) What time you would like to come.
